best possible results.  Use a black "Uni-ball" pen.  This pen produces
the best results.  Practice writing your signature pressing firmly to 
get a dark, consistent signature. When finished practicing, please 
sign twice in the area below.  Keep your signature between the lines.
Please DO NOT let your signature touch the edges of the box.

Fill out the order form.  Mail this entire form with advanced payment
or credit card information to the address below.  Be careful not to
fold the paper where your signature is.  Once your signature is
converted to a TrueType font, we'll send it back on diskette, with
easy installation instructions.
